Urgent Care Centers to utilize your plan
benefits. Benefits do not apply anywhere
else. All visits to Physicians Immediate
Care are covered under your plan with
only a $20.00 utilization fee per visit.
Scope of Services:

All general medical care (injuries,
acute illnesses, most covered by a
general medical practice)

Minor Emergencies

Repair of cuts and wounds

Lab draws

Access to generic medicines

Splinting/bracing for sprains,
strains, and fracture care

Sports medicine

Various physicals including school,
sports, work, and general yearly
physicals
Membership Fee:
The cost of the plan is

Individual Coverage is
$49.00 per month for individual coverage.

Family coverage is $49.00 for
the first individual and $39.00 for
each additional family member per
month.
Membership Qualifications:
- A
checking account in good standing is
required for membership.

Fees will be automatically deducted
at the beginning of each month.
- A
minimum six month contract is
required.
- A
sixty day written notice of
cancellation is required.

First and last month's payment are
due upon inception of plan.
